Donation update form

You need to register before updating your blood donations. We must apologise that the process is not more automated but we will get there in time. Once you have submitted the registration form we will allocate you a membership number which you will use when you update your blood donations using this form. In the future, you will be able to view the status of your recorded donations and your membership points that you have earned. Please be patient until then.
